REPAIR AUTHORIZATION AND AGREEMENT
- I hereby authorize Carrollton Body & Paint Shop LLC d/b/a Carrollton Collision Center (Hereafter referred to as the shop) to perform repairs on the vehicle described on the link below and to operate the vehicle for the purposes of testing, inspecting or delivery at my own risk. I agree that the shop is not responsible for the mechanical condition, loss or damage to the vehicle (or the contents) caused by fire, theft, accident, or causes beyond our control.
- Old and or damaged parts removed from the vehicle will be disposed of.
- I understand that storage charges of $25.00 per day will be assessed on vehicles that are not repaired or are considered a total loss.
- I understand that I am soley responsible for payment of any "deductible, depreciation or betterment" that may apply.
- The shop reserves the right to require that all repair charges be paid in full before the vehicle is released. It is my responsibility to ensure that Carrollton Collision Center receives payment prior to the release of my vehicle. If an insurance claim and my vehicle is released to me prior to full payment of the final repair amount, I understand that I may be responsible for the balance of my account in full.
- If payment has not been received and the vehicle has not been picked up after 45 days from the completion of repairs, the vehicle becomes the property of the shop.
- To ensure payment for repairs, an expressed mechanic's lien on this vehicle is acknowledged, and I also agree to pay reasonable attorney's fees and costs in the event that legal action becomes necessary to enforce this contract. In some instances the shop may ask for your socal security number.
- I understand that the shop accepts for payment cash, and personal checks in addition to Visa and MasterCard; however a convenience fee of 3% will be added to the amount charged to my credit card.
